Sub: Request to change my name to my new legal name in academic record

Respected Sir,

I am Jyoti Mishra, Roll No-RQ1939B4, Reg. No.-11910540, pursuing my Master of Business

administration (MBA) in Mittal School of Business, LPU Jalandhar. With due respect, it is to
be stated that after my marriage with Arvind Kumar Pathak, my name has been changed from
“Jyoti Mishra” to “Jyoti Pathak”. I am now legally using the name Full Married Name
“Jyoti Pathak”.
As per Indian Law, the following documents are required for a name change.
1. My marriage is registered under the Hindu Marriage Act, 1955 or the Special
Marriage Act, 1954 and my marriage certificate vide registration No 0397 /
SATN/0000594/8000086607/2018 dated 14/06/2018, is attached herewith.
2. An affidavit vide AU 963969 dated 30/12/2019 for change of name after marriage,
intended by me, is attached herewith.
3. Advertisement in Hindi and English language for name change in the local newspaper
dated 14/01/2020 and 19/01/2020 respectively, is also attached herewith.

Therefore, I request you to change my name from “Jyoti Mishra” to “Jyoti Pathak” on all
the academic records of the college so that it didn’t cause any problem in future.

I will be highly obliged to you for the same.
