AFFIDAVIT OF UNDERTAKING
I, MARICAR ODIAMAR, of legal age, Filipino, and a resident of Roberto
Sobrepeña St., Monte Cielo Del Rosario, Naga City, Camarines Sur, after
having been duly sworn to in accordance with law hereby voluntarily say:

That I am the registered owner of a Toyota Fortuner with plate number

EAB-1285;

That on December 11, 2020, the said vehicle figured in a vehicular

accident in Brgy. San Felipe, Basud, Camarines Norte, where one Haine R.
Pascuhin perished as a result thereof;

That the said vehicle is now under police custody as a result of its
involvement in the accident;

That the vehicle is bound to deteriorate in its present situation, to my

damage and prejudice;

That all parties involved in that accident have already reached a

compromise agreement and will no longer file any case in court;

That consequently, I am requesting the police to release the said vehicle

in my favor in view of the settlement arrived at by the parties involved;

That I hereby undertake that in the event that the aforementioned

vehicle will be needed in any investigation/case/proceeding, I will personally
bring and make available the said vehicle before any court, office, or body
requiring its presence;

That I will likewise submit any and all documents which may be
reasonably required by the proper authorities for the release of the said vehicle;

IN WITNESS WHEREOF, I have hereunto affixed my signature this 12th

day of December, 2020, at Daet, Camarines Norte.
