Re: Reiteration of request to delivery UMID card

Dear Ms. Villavicencio:

This is in reference to my request for delivery of my UMID card since the year 2017. This is
already my second letter-request to your office relative to the said delivery of my UMID
card which was apparently delivered at the LandBank – Iloilo Branch.

As a point of emphasis, I am simply requesting your office if you could facilitate the
delivery of my UMID card from Landbank - Iloilo to your branch. Thus, I am imploring
your good office to help me in this request in view of the fact that it has already been
three (3) years since it was process. Attached herewith a machine copy of previous UMID
card issued by Union Bank and copy of EMV-enabled card claim form from Landbank.

Accordingly, this is a big dilemma for me since I could not practically avail of the services
of GSIS thru my UMID card especially in this pandemic season. In view thereof, I am
imploring that you may act promptly in my simple request. I would be a great blessing if
you could delivery my UMID in the soonest possible time.
