Staff Task Checklist

Haypath House, MHANC CLP

Daily Routine Tasks:


 Upon Arrival
o Task Checklist (this document): review; complete stated tasks
o Staff Communication Log: read and attend to stated matters
o Walkthrough: inspect house (common areas and bedrooms), complete
Walkthrough form (especially identify potential hazards and prompt residents to
remedy)
o Calendar: review for upcoming appointments, prompt residents to schedule
transportation reservations; make Appointment Packets (Medication List; copy of
Medical Insurance Cards, if the provider is new; Referral, if necessary; other
relevant documents if requested)
o Email: read and attend to stated matters
 At Appropriate Time
o Controlled Substance Count: 8 AM or upon arrival
o 8 AM Meds: monitor
o AM Blood Glucose Testing: monitor and record
o Chore Time: 12 PM; See Chore List and Kitchen Clean-Up List; prompt
respective residents
o Resident Mail: important mail to be opened with a counselor; place the rest in the
mailboxes
o 2 PM Meds (if applicable): monitor
o Controlled Substance Count: 3 PM or upon arrival
o Dinner: prompt respective resident to start preparing dinner
o Chore Time: 6 PM (especially, prompt appropriate resident to clear the dryer’s
lint trap and to bring garbage and recyclables to the curb on the respective
evenings)
o 8 PM Meds: monitor
o PM Blood Glucose Testing: monitor and record
o Controlled Substance Count: 10 PM or upon arrival
 Overnight
o Stove Knobs: remove for the duration of the night
o Doors: lock Basement and Entrances
o TV: turn off and prompt residents to sleep
 As Needed
o Refill Calls (Pharmacy or Prescriber): prompt or assist resident

Weekly Routine Tasks:


 Med Packing: monitor
 Spot Checks: inspect the packers of residents self-holding meds, randomly 2x+/week

 Goal Sessions and Goal Session Progress Notes: conduct and submit, print, sign and file
its Progress Notes section of chart (the first one by the 7th of the month, the second by
the 11th, the third by the 18th, the fourth by the 21st)
 Med Counts
 Food Shopping
 Recreation Trips

Monthly Routine Tasks:


 Fire Drills
 Clozaril Blood Monitoring Testing
 Collateral Contact Note in Foothold
 Print upcoming month’s Medication Monitoring forms

Quarterly Routine Tasks:


 Service Plan Reviews (SPR’s)

Semi-Annual Routine Tasks


 Physician Authorizations (PA’s)

Yearly Routine Tasks:


 Chart Documents: update (see the starred documents listed in the table of contents,
“Resident Chart Contents,” in the front of each chart); e.g., Self-Preservation Test,
Consents, General Physical (or Declination), PPD/Chest X-Ray (or Declination), Dental
Exam (or Declination), Gynecological Exam for women (or Declination), Podiatry Exam
for diabetics (or Declination), Ophthalmology Exam (or Declination), WRAP/Crisis Plan,
Emergency/Disaster Plan
