PREFACE
This Petrofac Health and Safety, Security, Environment and Integrity Assurance Management
(HSSEIA) Standard describes the expectations and performance requirements to be met with
regard to risks to health and safety, threats to security, aspects of our environmental impact and
assurance of the integrity of our assets. This document sets out the approach taken to
implement the Petrofac Limited Policies for HSSEIA.
This standard has been approved by the Petrofac Group Director HSSEIA and is mandatory for
all Petrofac Limited subsidiaries, Joint Ventures where Petrofac have control and contractor

activities insofar as these are referenced in the contract.

Each Petrofac division and business unit is required to adopt this standard as a mandatory
requirement. Guidelines, tools and supporting information relating to this standard are provided.

1.1 Purpose

• provide structure for the implementation of the above documents with regards to

• define the accountabilities, roles and responsibilities for implementing permit to
work systems across Petrofac

• provide a consistent approach to the implementation of permit to work systems
for the management of risks associated with work activities across Petrofac

effective processes in place to manage risks arising due to implementation of
permit to work systems across its business activities

2.0 APPLICABILITY
In the event of any conflict or contradiction between the provisions of this document and
local legislation the applicable laws must be met.
The approval authority for management of change or any request for dispensation
relating to any of the expectations or performance requirements of this standard is the
Group Director HSSEIA.
This document shall be reviewed by the Group Director HSSEIA, as a minimum every
three years to ensure it remains fit for purpose and effective.

cases, they can also be referred to as the competent

These are the minimum expectations and performance requirements placed upon
Divisions and BUs. Brief advice on compliance with the standard is given within the
document.
There is always a potential for a serious accident whenever oil and gas activities are
performed, given the large quantities of flammable and toxic materials being handled. To
prevent them, it is vital that adequate control of work system is implemented. The permit
to work is an essential part of this control of work system.
The permit to work determines how the job could be carried out safely. It is not a
statement that all hazards and risks have been eliminated from the work area. The issue
of a permit does not, by itself, make a job safe. Safety on a job can only be achieved by
those preparing for the job and those carrying it out.

4.1 Golden Rules of Safety – Permit to Work

BUs shall adopt the Golden Rules of Safety to ensure safe implementation
of permits to work.
Before conducting work that involves confined space entry, work on energy

systems, ground disturbance in locations where buried hazards may exist, or hot work in
potentially explosive environments, a permit must be obtained that:
• defines scope of work
•

identifies hazards and assesses risk
establishes control measures to eliminate or mitigate hazards

• links the work to other associated work permits or simultaneous operations
• is authorised by the responsible person(s)

4.2 Planning
Permit to work shall be implemented as part of the overall control of work process. A

permit shall be considered wherever it is intended to carry out work which may adversely
affect the safety of personnel, the environment or the plant (i.e. any potentially
hazardous work). Such works include but are not limited to:

In addition, the permit to work system shall ensure that all necessary precautions such
as energy isolation, provision of access barriers, etc. are identified by a task risk
assessment before any work is undertaken.
Works requiring permits shall be carried out as part of the overall control of work
process. As such, the steps involved may already be present but shall be part of an
interdependent system that meets with requirements of the Control of Work Standard
(PL-STAN-HSSEIA-013).
As part of the planning process, the permit shall be accompanied by a risk assessment

used to identify all hazards associated with the work. The risk assessment shall be
conducted in line with the requirements of the HSSEIA Risk Management Standard (PL-

STAN-HSSEIA-009). The risk assessment shall identify and control:
• both site and task specific/process hazards
• hazards associated with the task which may impact on the surrounding

environment/site
• the potential for other activities to impact upon the task
The permit shall contain details of all necessary precautions which shall be in place prior
to the work taking place.

The Performing Authority shall ensure that all relevant precautions set out in the permit

are in place before the work begins.
Where there is a requirement to remove safety critical equipment from service, the
permit to work system shall ensure that:
•

all relevant parties, including the safety critical equipment’s responsible persons,
are notified and, where required, approve the work before it starts
• the potential effects of removing such equipment from service shall be fully
assessed, as part of an ORA, if required, and that all identified controls and
contingency measures are applied

4.2.4 Interaction
U
Every site shall have a process in place to formally assess and control risks associated
with simultaneous operations (SIMOPS). This process shall:
• include the identification and management of interfaces between working parties
that have a potential to impact each other
• ensure that permits for conflicting activities are not issued simultaneously
Where a number of permits are active simultaneously, the Issuing Authority shall ensure
that all interfaces are effectively managed.
Work activities requiring a permit shall be planned and co-ordinated in order to avoid
risks caused by combined operations or simultaneous activities. A permit co-ordinator
shall be appointed to control the issue and return of all permits to work.
It is important to make sure that one activity under a permit to work does not create
danger for another, even if the other work does not require a permit to work. The Issuing
Authority shall be aware of potential interaction and shall ensure that when a permit is
prepared, the work to be carried out takes account of other activity currently planned or
underway.
It may be that the interacting activities are covered by separate Responsible Authorities,
in which case close liaison shall be necessary, e.g. through cross-referencing on the
permit, the task risk assessment or in the work pack.
If an isolation is common to more than one work activity then it shall be clearly detailed
and an isolation certificate attached to each associated permit. The isolation shall be
separately locked out by each relevant Performing Authority.

The permit to work system shall have specific permit types for the following categories of

work: Hot – Naked Flame; Hot – Spark Potential; Cold; Cold – Breaking Containment.
Each permit shall contain or refer to the following information (not exhaustive):
• description of the task to be done

• exact description of work location
• exact description of plant/equipment to be worked on including the
plant/equipment number

details of the work party and tools to be used
details of potential hazards and precautions taken

• details of protective equipment to be used or worn
• reference to any required supporting documentation (e.g. method statements,
drawing, etc.)

Energy Isolation
Any work that will be undertaken that requires isolation (mechanical, electrical, process,
etc.) shall have a supporting isolation certificate attached to the associated permit. If this
is not in place the work shall not proceed. All energy isolations shall be planned and
conducted in line with the requirements of the Energy Isolation Standard (PL-STAN-
HSSEIA-003).
Where work is being carried out under a permit with an associated isolation, the Isolating
Authority must sign off the isolation certificate and associated permits. The Performing
Authority however is responsible for validating that the isolation is in place and adequate
prior to commencing the work.

Confined Space

Any work to be undertaken in a confined space shall have a supporting confined space
entry certificate attached to the associated permit. If this is not in place the work shall not
proceed. All confined space entry work shall be planned and conducted in line with the
requirements of the Confined Space Entry Standard (PL-STAN-HSSEIA-001).

Ground Disturbance
Any work that involves digging, excavation or boring shall have a supporting ground
disturbance certificate attached to the associated permit. This certificate which is also
known as excavation or heavy equipment movement certificate ensures that no

underground services or pipework is affected. If this is not in place the work shall not
proceed. All works involving ground disturbance shall be planned and conducted in line
lle
with the requirements of the Ground Disturbance Standard (PL-STAN-HSSEIA-002).
Gas Testing
Any worksite where there is likely presence of flammable or toxic gases or oxygen

deficiency or enrichment shall have a supporting gas testing certificate attached to the
associated permit.
A tool box talk section shall be included within the permit. Prior to the start of work, the
Performing Authority shall conduct an onsite tool box talk with all members of the work
party to ensure that they are aware of all the hazards and details of the work and the
permit. The tool box talk shall be signed by all participants. The permit shall be updated
to reflect any additional hazards and control measures which have been identified.

4.3.3 Authorisation
The Permit to Work system shall ensure that the Issuing Authority and the Performing

Authority is not the same person for any permit. All permits shall be authorised by the
Issuing Authority. Those performing authorisation of permits to work shall be suitably
qualified and competent to do so.
4.3.4 Display
Whilst the work is in progress, the Performing Authority and the permit users shall be

aware of the requirement, to stop the job if at any time they feel that:
• it is unsafe to continue

• one of the control measures dictated by the permit is noted as not being in place
• there is any change to the work which needs to be managed

4.3.6 Suspension
Where work being carried out under a permit is stopped before completion, the permit
shall either be suspended or cancelled. In such cases appropriate controls shall be in
d
place to ensure the integrity of the equipment and the safety of all site personnel.
Permits must not remain suspended beyond their validity period and a suspended permit
lle
shall remain live until it is cancelled. Suspended permits shall remain on the permit
recording system. In addition, the condition in which the plant has been left and the
consequences for other activities shall be specified.
In the event of an emergency, all work shall cease and the permit shall be suspended.

When a permit is suspended or cancelled, the work should not be restarted until the
Issuing Authority/Area Authority have verified that it is safe to do so and has re-validated
the permit or issued a new permit.

4.3.7 Re-validation
A permit shall only be issued for one shift with a maximum period of 12 hours. If work is
required to continue, the permit can be re-issued on an on-going basis but only up to its
validity period. The validity period of any permit shall not exceed 14 days.

When a permit is to be re-issued for a new shift, the two associated Performing
Authorities shall be required to visit the work site together and clearly communicate the
status of the activity. A shift handover procedure shall be in place to support the Permit
to Work system.

Upon completion of work, the permit shall be returned to the point of issue. Both the
Issuing Authority and the Performing Authority, after a site inspection, shall sign to
indicate completion. Other parties to the permit, i.e. those made aware of the work who
may have been signatory to the permit, shall also be informed.
Returning plant/equipment, which was under permit to work, back to service shall only
proceed if:
• the work on the equipment has been completed
• the plant/equipment has been left in a safe condition and that this has been
verified by the person signing off the permit
•

all isolations/overrides pertaining to the plant/equipment have been removed or
the status of any remaining isolations/overrides is known to operations personnel

• the operations person responsible for that area formally acknowledges his
acceptance of the plant/equipment

Appropriate document control process shall be applied to all permits to work. Permits
shall be retained for a minimum of 12 months.
The permit to work system shall clearly identify all training and competency requirements
for each defined role. These shall be met by all personnel, as per the specific role that
they fulfil within the permit to work system.
