I, ARLYN CASIMERO BRILLANTES, of legal age, Filipino
Citizen, single and a resident of Sitio Iba, Basak, Lapu-Lapu City, after having been sworn to in accordance with law, hereby depose and say:
1. That I am a holder of a Philippine Passport duly issued by
the Department of Foreign Affairs. The details of the said passport I can no longer recall;
2. That the said Philippine Passport expired while I was in
Kuwait working as a Domestic Worker;
3. That when I decided to leave my employment and return to
my mother country, the Philippines, my employer withheld the said Philippine Passport to prevent me from going home;
4. Reluctant to stay, I still went home and used my Travel
Document with no. 00249-2015 issued by the Embassy of the Philippines, State of Kuwait;
5. Considering that my Passport is not physically with me and
such is already expired, I am opting to renew the same for immediate travel abroad for employment;
6. That the said passport has not been encumbered, sold,
pledge, assigned, alienated, transferred, donated, or given as collateral for any debt or obligation to or in my favor of any person or entity;
7. That I hereby assume full criminal, civil and administrative
liability in case of any action, charges, suit, proceeding, prosecution or investigation that may be filed against me or any person by any court of justice, tribunal, board, agency, quasi-judicial body or office should any of the document or paper I submitted be found dubious, spurious or fake, or should any of my statements and declarations be found to be false of unfounded; 8. That I am executing this Affidavit to attest to the truth of the forgoing statement and for all legal purposes it may serve.
