Q2.apical periodontitis secondary to treatment

Sever tenderness to percussion immediately after the treatment initiated ,regardless to the
. condition before therapy

Some s & s as apical periodontitis

.throbbing ,localized pain.1

.tenderness on eating ,chewing.2

.tooth is slightly elongated.3

.vitality test :no response as the pulp was removed .4

Causes of secondary periodontitis

over instrumentation.1

due to wrong tooth length determination -

overmedication.2

a.too caustic

b.too much intracanal medicament

: pushing the debris or micro-organism to the periapiacl region .3

e.g in case of pulp necrosis

diagnosis (test to confirm whether apical tissue has been over instrumented)

rubber dam placement ,sterile paper point is selected

if the apex is over –instrumented

the point will extends easily past the working length .on withdrawal ,the tip of point will .
disclose a reddish or brownish

Emergency ttt : tooth is opened & W.L is readjusted

Canals re_cleand & irrigated with NAOCL  canal is medicated with CA(OH)2

Strong analgesic +corticosteroid .antibiotic medication

the antibiotic is to prevent any possible overgrowth of micro-organism that might proliferate (
when the inflammatory response is lessened 0 .put the tooth out of occlusion
