1. Reverse Transcription – Polymerase Chain Reaction (RT-PCR) Negative

Result (test must have been undertaken 3 days from the date of travel)
2. Medical Certificate indicating medical condition/ Referral Note from the
Hospital/ Confirmed Hospital admission
3. Laboratory Request issued by the attending physician
4. Valid Identification Card

Government APOR on Official Business

1. Medical Certificate from the City Health of Origin

2. Government/ Agency ID
3. Mission Order issued by the Department Head

Authorized Person Outside of Residence (APOR)

1. RT- PCR Negative Result (test must have been undertaken 3 days from
the date of travel)
2. Work Travel Order / Mission Order issued by the Department Head
3. Company ID
4. Confirmed Booking of Accommodation
VISA/ Embassy

1. RT- PCR Negative Result (test must have been undertaken 3 days from
the date of travel)
2. Confirmed VISA Interview/ Appointment
3. Valid Identification Card
4. Return Trip ticket/ Travel details
5. Confirmed Booking of Accommodation

Locally Stranded Individual

1. RT- PCR Negative Result (test must have been undertaken 3 days from
the date of travel)
2. Letter of Coordination from the Barangay in Cebu City
3. JTF COVID Shield Travel Authority (Medical Certificate, Barangay
Clearance from LGU of Origin)

Cadaver Transfer

1. Death Certificate of Cadaver (Non-COVID)

2. JTF COVID Shield of All Accompanies
3. RT- PCR Negative Result (3 days from the date of testing until the
scheduled date of arrival)

Non – Essential Travel (Humanitarian, Leisure/ Vacation, Others)

1. RT- PCR Negative Result (test must have been undertaken 3 days from
the date of travel)
2. JTF COVID Shield Travel Authority
3. Return Trip ticket/ Travel details

 RT-PCR / Swab Test Negative Result (valid 3 days from the date
of testing until the scheduled date of arrival)
