No: 03405647047do hereby undertake on this the 05 (Day), of June (Month) 2020 (Year), the following:-

1. I, hereby, declare that, the entries made by me in the Application Form are complete and true to the
best of my knowledge and based on records.

2. I, hereby, undertake to present the original declaration from immediately to the concerned
authorities of the management of Phalia International (Private) Limited.

3. I, hereby, promise to abide by the admissible rules and regulations, concerning discipline, attendance,
etc. of the Phalia International (Private) Limited, and also to follow the Code of Conduct prescribed for
the Students of the Phalia International (Private) Limited, as in force from time to time and subsequent
changes/modifications/amendment made thereto. I acknowledge that, the Institute has the authority
for taking punitive actions against me for violation and/or non-compliance of the same.

4. I understand that, all classes are online and 100% attendance in classes is compulsory and I commit
myself to adhere to the same. I will be bound to appear in international certification exam (if applicable)
within 3 weeks after completion of training for obtaining certification from the concern body, in case my
attendance falls short for any reason or I am unable to appear in the certification exam, I will be liable to
pay 100% training/certification cost to the Phalia International (Private) Limited.

5. I, hereby declare that, I will not join more than 02 trainings/certofications from any partner
orgnzation under this scheme. If found, than I will be liable to pay 100% training/certification cost of all
the trainings/certifications to the Phalia International (Private) Limited.

6. I, hereby undertake to inform the Institute, about any changes in information submitted by me, in the
Application Form and any other documents, including change in addresses and phone nos., from time to
time.
