JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MERLITA J. TIMOG and NELSON B. CAHAYAG, both Filipinos, of legal age and residents of
Malitbog, Southern Leyte, Philippines, having been duly sworn in accordance with law, hereby depose and
state:

That we both personally know the person of BERNARDINA C. JACOBA because his family
has been a close family friend;

That as per record in her Death Certificate her name was recorded as BERNARDINA C.
JACOBA;

That as per record in her OSCA ID her name was recorded as BERNARDINA C. JACOBA;

That as per record in her daughter’s birth certificate her name was recorded as BERNANDINA
CANTA;

That as per record in her DSWD Payroll her name was recorded BERNADINA CANTA
JACOBA;

That the name BERNARDINA C. JACOBA as per record in her Death Certificate, the name
BERNARDINA C. JACOBA as per record in her OSCA ID, the name BERNANDINA
CANTA as per record in her daughter’s birth certificate, and the name BERNADINA
CANTA JACOBA as per record in her DSWD Payroll, refers to one and the same person;

That the reason for the discrepancy of her name was due to the excusable negligence and/or
inadvertence of the person supplying the entries back then;

That we are not related in any way, either by consanguinity or affinity, to the said
BERNARDINA C. JACOBA;

As such, we execute this Joint Affidavit to attest to the truth of the foregoing facts and for any
other legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 23rd day of October 2020, at Tomas
Oppus, Southern Leyte, Philippines.
