AFFIDAVIT OF DISCREPANCY

           I, VICTOR M. GONZALES, of legal age, Filipino, and a resident of Brgy.

Cabitan, Mandaon, Masbate, after having been duly sworn in accordance with law, do
hereby depose and state:

1. That I was born on April 7, 1986 in Manila to parents Leopoldo L.

Gonzales and Lourdes Madrelejos;

2. That I have been publicly known by the name Victor Madrilejos

Gonzales, as evidenced by my government issued identification cards;

3. That however, just recently, when I secured a copy of my Certificate of

Live Birth from the Philippine Statistics Authority, I came to know that
the spelling of my name as appearing therein is Victor Madrelejos
Gonsalez;

4. That also, in the Marriage Contract of my parents, my mother’s

surname, as erroneously appearing therein is Madridejos, when it
should have been Madrelejos, same with what appears in my Certificate
of Live Birth;

5. That I am currently requesting for the correction of the erroneous

entries in my Certificate of Live Birth, particularly as to the spelling of
my surname, from Gonsalez to Gonzales, and further, from now on, I
shall be using the name Victor Madrelejos Gonzales;

6. As such, we execute this Joint Affidavit to attest to the truth of the

foregoing facts and for any other legal purpose that this Affidavit may
serve.
