CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENT:

This CONTRACT executed and entered this _____ day of _________, 2020
at Cabarroguis, Quirino by and between:

The PROVINCIAL GOVERNMENT OF QUIRINO, a political subdivision of the

state with business address at the Provincial Capitol, Capitol Hills,
Cabarroguis, represented herein by its Governor, the HON. DAKILA CARLO
E. CUA, herein after referred to as the FIRST PARTY;
and
JOVINO T. ARCEBAL, proprietor of ARCEBAL AUDIO MOBILE LIGHTS AND
SOUND & ENTERPRISES, with residence and postal address at Maria Clara,
Diffun, Quirino, hereinafter referred to as the SECOND PARTY;

W I T N E S S E T H:

WHEREAS, the FIRST PARTY has signified its interest to hire a reliable and
durable sound system with complete accessories to be used during the CARAVAN
in different barangays of the province which will be held on
__________________;
WHEREAS, in this connection, the SECOND PARTY has offered his sound
system for the purpose stated;
WHEREFORE, premises considered, the PARTIES hereto agree and covenant
follows:
1. That for and in consideration of the amount of FIVE THOUSAND FIVE
HUNDRED PESOS (P5,500.00) per barangay as venue for CARAVAN, to be
paid by the FIRST PARTY, the SECOND PARTY undertakes to provide the
sound system with complete accessories;

2. That in case where the sound system, accessories or parts thereof be

damaged, the SECOND PARTY shall be held solely responsible thereto
absolving the FIRST PARTY from any obligation;

Contract between PGQ and JOVINO T. ARCEBAL for hire of a Sound System for the conduct of CARAVAN 2020 in the amount of
FIVE THOUSAND FIVE HUNDRED PESOS (P5,500.00) per schedule/day.
 IN WITNESS WHEREOF, the PARTIES have hereunto affixed their respective
signature on the date and place first mentioned above.

ACKNOWLEDGMENT

BEFORE ME, Notary Public, personally the PARTIES both known to me and
to me known to be the same persons with gov’t. Issued id who executed the
foregoing agreement and that they both acknowledged to me that same is their
free and voluntary deed.

This document consists of two (2) pages including this page where the
acknowledgment is written and signed by the parties and their instrumental
witnesses.
