For good and valuable consideration, the receipt and legal sufficiency of which is
hereby acknowledged, I (on behalf of the “Model”) hereby grant to the Contributor and Country Postal code
SHUTTERSTOCK, INC. (collectively, the “Contributor”), the Contributor’s assigns, and
those persons acting with the Contributor’s authority and permission, the right to take Email Phone
and create photographs/videos and other still and/or moving images (in all formats)
and other graphical depictions incorporating the Model’s likeness, in any and all Signature Date
media, whether now known or hereafter created (the “Content”).
I hereby agree that all rights in and to the Content, including the copyright, are and Notes (optional)
shall remain the sole property of the Contributor, free and clear from any claims by me
or anyone acting on my behalf.
Model information
I understand and agree that the Contributor’s and the Contributor’s assigns’ rights
include, but are not limited to, the rights, in perpetuity, to:

• Alter, modify or otherwise change the Content in any manner the Contributor
desires or permits;

• Index and analyze the Content, which may involve extracting scans of the
Model’s image, likeness, facial geometry and other biometric data as may be
derived from the Content; Parent/Guardian information
• Combine the Content with textual matter and/or with other pictures and/or
media; and, Name (print)

• Use the Content for illustration, promotion, art, editorial, advertising, trade,
publishing, or any other purpose whatsoever.
Relationship to model

I acknowledge and agree that neither I nor the Model shall have any further right to Address
additional consideration or accounting from any sublicensing, distribution, publication,
and/or further uses of the Content and that this license and release allows the use of City State
