Please find the report on inspection carried out , submitted here with for your
reference:-
The inspection was carried out on March 27,2014, Between 10 to 14 hrs, to
assess the H&S standards compliance and effectiveness of implementation of H&S
Management system of Crystal Cosmetics Private limited , located at Hyderabad . The
company is engaged in the manufacture of consumer produts like talcum powders and
tooth powders. The Manufacturing facility is situated in Industrial estate .
It has got a total strength of about 100 employee, 76 workmen and 24 staff.
[In some observations may find the word facility meaning the company].

The Facility has got lot of work equiment, handles sizable number of chemicals and
some hazardous substances.The operations include Mixing of ingredients of formula
quantities and filling and packing in metal cans or containers.

During the Time of inspection manufacturing activities were on in full swing and hence
could get adequate number of observations, out of which few deviations or departure
from standards are of serious nature. Timely actions shown against respective
observations will ensure substantial improvement in the company ,becoming one of the
safe and best place to work

There are risks of Moderate nature like dust hazards, noise hazards ,vehicle movement ,
Hygiene and sanitation health risks as required under convention C120 on Hygiene
1964 . All these also require timely action .

There are system related issues concerning contractors as per ILO-OSH-2001 3.10.5.1
recommendations which need to be addressed.

Main findings of the inspection


Company has attempted to provide good work environment , Plant and equipment with
safety features
Inspection however has revealed shortcomings .which are described below
Work equipment : Observations 1a, 1b, 21 on exposed movable parts [some moving at
even high speed] was observed. Removal of fixed guards fall under permit to work
sytem , of H&S MS
Recommendations ;Hence it is recommended to replace with new durable stainless fixed
guards ensuring no gap or access is left out. Should cost approximately INR 2,40,000

Regulations : under R118 of ILO -1963 also Indian factories act 1948.and is found to
have been violated.
Observation no 16 on interlock guards malfunctioning observed with potential injury by
contact with moving parts . Wilful overriding of interlocks is punishable .
Recommendations: The non working limit switches replenishment with good quality
ones should help.. Reenforcement of LOTO [lockout tagout] by providing new LOTO
locks and strict monitoring will ensure good system.

Hazardous substances/wastes storage and handling:


Observation nos 11, 28 in Inspection has shown unsafe storage conditions , Handling of
chemicals without appropriate PPEs.Lack of awareness of hazardous substances ,in the
form of raw materials , inprocess material , finished products and wastes of hazardous
substances.
Recommendations: Training to the cocerned stores staff and contractors will help
improve , and prevent undesired exposure to chemicals. In addition improvement in
condition of storage with adequate movable space and arrays , proper provision of
secondary containemnt to liquid items are area of importance. Approx cost totally works
out to Inr 400000 or 6500 USD
Regulatory requirement: OHSAS 18001 standards.There is violation of Environmental
Protection act 1986 and hazardous substance and waste storage and handling rules
1989] promulgated by governement of India.
Electrical Hazards: Observations no 8, 23 of unauthorised entries ,Leaving Many
electrical panels and DBs for easy access can pose a seroius Risk .
Recommendation: Provision of securing these areas ,lock and key arrangements entry
to only authorized licenced electrical workmen. Leaving such Control panels open to
atmostphere will endanger in the event of rains and cyclones.Fixing the covers firmly
and securely is of Importance. Approx estimate of cost INR65000, or 1100 USD

House Keeping: Observations no 12,14 on house keeping shows , the major


contribution is from behavior, unsafe practice and importance not given to House
keeping.. severe risk , as inspection shows possiblity of topling of stored metal items or
scraps .There are few work areas , as the fabrication areas and Engineering workshop
where items are kept haphazardly posing risk of trip and fall .
Recommendations: A 5S sytem if implemented should solve the problem with space
mangement . Adequate manpower resource or contractors needed to set things right.

Noise: Observation 20 is revealing significant bearing on the working. Compounded


noiselevel has been high in the packaging or filling area , which calls for combating the
risk at source to minimize it at the generation point generating above 82.5 dB, actionable
limit as per Niosh is 80dB.There is Vibrating sytem for feeding of lids and can feeding
station all giving rumbling noise.
Recommendation:Damping method using ,like padding and cuishioning or providing
accostic enclosures suitably fabricated will considerably minimize the problem. Annual
health servillience is a must for all exposed to noise .

Dust : Observations 9 and 24 point out , the generation of dust and inadequate control
measures and is obvious in the manufacturing and filling areas:
Recommendations:Local exhaust ventillation, Provision of new Efficient dust extraction
systems for SLS handling and weighing strongly recommended . Those which are
provided need to be , periodically monitored and improved.Inspections of LEVS,
blowers, filters and maintenance and measurements should ensure improve
effectiveness. Use of PPEs are must as additional line of defence for both noise and
dust.

Health , Hygiene and sanitation:


Drinking water arrangement have some health risk ,if not inspected and the purifying
systems and filters not cleaned and maintained periodically. One of the basic
requirement of welfare Regultion ILO c120-1964

First Aid: inspection has identified lack of supervision, and timely replenishment of first
aid items in the first aid boxes. Expired items need to be scrape.
Vehicle movement and controls: has got exposed in the inspection for lack of control
of the sytem, supervision and appropriate effective security control should minimize this
problem
Contractor Management: Observations no 13 and 26 is a H&S Mangement system
deviation from selection to engaging and monitoring, couple of instances have revealed
either lack of awareness of contractors e,g handling of hazardous substances and not
using appropriate PPEs and careless and negligence .
Recommendations : H&S Selection criteria be followed as 3.10.5.1 of ILO-OSH-2001,
and awareness training on risk , SSW to be complied with and supervision can avert
likely mishaps..

Conclusions:
Well, the Observations and main findings of this inspection and actionable control
measures in this report will help significantly and substantially improve the H&S
standards and H&S management system. Roughly about 40% of the inspection
observations are conditions related and about 60% approximately behavior oriented.
Observations cover vide range of hazards from low to High risk.There have
been some good practices which is appreciated,It is advised that the recommendations
included in this report are implemented in order to improve health & safety and aid legal
compliance. All improvements should be monitored and reviewed to establish effectiveness.
Addressing these issues exercising control measures and implementing them is the
key.
Implementation of recommendation does involve cost and is expected of manageable
level, considering the cost company may incur in the event of any accident or incident
I shall have to re iterate there are areas of concern as mentioned in the main findings.
Reviewing the risk assessments at areas of hazards , periodic inspections ,
internal audits site tours and tracking these with timely actions ,will go miles in
improvement and ensure continual improvement

Management has shown keen interest by giving me an opportunity for conducting


inspection with an intention and commitment for improvement.Implementing the
recommendations given will be really satisfaying for me .We shall follow it up with
