BASIS FOR THE RECOMMENDATIONS IN A CONSULTING SERVICES FAILURE

ANALYSIS REPORT

The Consulting Services Department final report is a result of the combined efforts of the
field and laboratory examinations and analysis. The report analyzes the field and laboratory
data, establishes the root cause of failure, and provides recommendations for replacement of
the failed part or parts and repair of associated damage. The report also provides
recommendations or procedures to avoid future failures. The general format for the failure
analysis report is as follows:

• Background, which includes field observations and information that is included

in the Failure Analysis Report Form.

• Examination, which summarizes the laboratory analysis.

• Discussion (optional), which provides additional information that is necessary

to make conclusions.

• Conclusion, which presents the root cause of failure.

• Recommendations, which provide repair and replacement procedures as well as

methods for preventing future failures.
