Subject: PHD SUPERVISOR CONSENT LETTER TO MR. IMRAN AJMAL
1. I wish to inform you that I am willing to accept you as my PhD student, to fulfill the
requirement of PhD admission application, in the subject of …………….........
2. This letter will be considered valid if you qualify the GAT Subject/IUB Departmental Test and
interview.
3. Admission shall be granted in accordance with HEC rules/regulations and IUB policy regarding
PhD admission.

4. Admission shall be granted according to the HEC policy of student-teacher ratio at the time of
last date of admission.
5. In case of violation of any of the HEC/IUB policy/regulations, this consent letter shall be
considered as withdrawn/cancelled.

6. You will provide the following documents for issuance of admission offer letter by the Office
of Advanced Studies & Research Board:-

(i) Two latest coloured photographs.

(ii) 01 attested photocopy of CNIC.
(iii) 01 set of attested copies of Academic transcripts, certificates and degrees.
(iv) 01 attested photocopy of Domicile.
(v) Attested photocopy of GAT Subject Result Card or Departmental Test Result.
(vi) Original NOC/Migration Certificate in case of last examination passed from the
university other than IUB.
(vii) 01 copy of NOC of the Employer Institute/Agency, if presently employed.
(viii) 02 letters of recommendations from previous teachers/professor.
(ix) Statement of Purpose.
(viii) Original Affidavit from candidate and parent/guardian worth Rs. 100/ each according
to IUB specimen
7. The detail of the students already working under my supervision is given vid attached
Proforma.
