AFFIDAVIT OF DISCREPANCY
I, VICKY CUSIPAG ARAO, of legal age, Filipino, and a resident of Minanga,
Penablanca, Cagayan, after having been duly sworn to in accordance with law, do
hereby depose and state:

1. That my full name is VICKY CUSIPAG ARAO, born on 22 FEBRUARY 1991, and the
fact of my birth was duly recorded before the Office of the Local Civil Registrar of
Peñablanca, Cagayan. Attached is a copy of birth certificate;

2. That since my childhood, and in all my public and private transactions, I used 22
DECEMBER 1990 as my Date of Birth;

3. Also, my Date of Birth that was entered before the Commission on Elections
(Comelec) is 22 DECEMBER 1990. Attached is a copy of COMELEC ID;

4. That when I secured a copy of my Certificate of Live Birth from the Philippine
Statistics Authority (PSA), I discovered that my true Date of Birth is 22 FEBRUARY
1991;

5. That I declare under the penalty of law that the names VICKY CUSIPAG ARAO born
on 22 FEBRUARY 1991 and the name VICKY CUSIPAG ARAO born on 22 DECEMBER
1990 refer to one and the same person, and that is me;

6. That I am executing this affidavit to attest to the truthfulness of the foregoing

narration of facts and for any legal purpose.
