    RE: REQUEST FOR MATERITY LEAVE

    Dear Sir,

    Good day!

    I am writing this letter to inform you of my pregnancy and to make a formal


    request for a maternity leave. My expected date of delivery is October 11, 2020. I plan
    to take my maternity leave starting from 1st day of September. I shall be able to resume
    my duty on November.

    In case I have to extend my leave due to any unforeseen event, I shall notify you
    well in advance.

    Thank you in advance for allowing me to take this time away so I can recover
    and spend time caring for my new baby.
