CHAPTER I

INTRODUCTION

Family is the basic unit of society. It starts with the two couple who will

eventually nurture a child that share a common emotional bond and perform social

tasks. They are also a source of strength, love and support for each other. Family

influences the health and activities of their members (Chen, Shiao & Gau, 2007). It

is important that families in the community are aware of the things and practices

pertaining to their health.

The status of each family will always affect the community. Community

health nursing have a goal of preventive, curative, promotive and rehabilitative care

for the community. This study aims to improve deficiencies and to give a holistic

care for the family that was chosen.

The data that will be provided will be an account of the nursing management

of one family in Barangay Butong who have a health problem. The family that was

chosen has a potential when it comes to health hazards in their environment that will

be discussed further in the following chapters.

The factors that affect the environment problems will be discussed as well as

the family nursing process which is the same nursing process as applied to the

family, the unit of care in the community and promotion of health.

The aim of the case study is to enhance the family’s knowledge about health.

The care delivered will be analyzed and ethical considerations will also be identified.

A rationale will also be given for the choice of intervention.

As community health nurses, we strive to promote prevention over treatment.

Our role as educators is to provide clients with information that allows them to make

healthier choices and practices. In order to improve individual, family, and

community health, correct knowledge, attitude and skills should be taught and

subsequently practiced. By correcting poor health practices and maintaining a safe

home or work environment, patients lower risks to their health and require fewer

CHAPTER II

OBJECTIVE OF THE CASE

This chapter presents the general and specific objectives of this family case

study. Setting objectives provides direction for planning a family nursing

intervention. It facilitates motivation for the client and the nurse by providing a sense

of achievement. (Kozier, Erb et. al., 2004)

GENERAL OBJECTIVE

After one month visit on Purok Uno Barangay Butong, Cabuyao Laguna;

we, the Second year student nurses of Pamantasan ng Cabuyao, College of Nursing

Group Nine, will be able to apply the learned knowledge and skills in assessing

family health status and prioritizing family health problems in the community for

health care, promotion and prevention. Also, the adopted family will be able to gain

knowledge with regards to improving their family's health status.

SPECIFIC OBJECTIVES

● Establish rapport with the family to develop a good working relationship

● Gather information about the family to support the case study

● Identify the family’s health condition as well as its history

● List down problems noticeable in the family

● Assess if there are presence of health threat, health deficit, wellness condition

and foreseeable crisis

● Promote health prevention of possible occurrence of disease

● Educate the family health teachings applicable to their problems

● Evaluate the results of the intervention taken, if the family improved their

condition

CHAPTER III

INITIAL DATA BASE

A. Structure Characteristics and Dynamics

The adopted family lives in Purok Uno, Barangay Butong Cabuyao, Laguna.

It is headed by the mother named Mrs. X. Her first husband died and she is currently

living with her new partner named Mr. X for eight years now. Among the six

children of Mrs. X to her first husband, the second to the youngest son and her

youngest son is living with her along with daughter-in-law and grandson.

The type of family structure is reconstituted according to the data acquired.

Mrs. X is sixty years old, she is the head of the family and has a role in decision

making when it comes to budgeting and health care. Her live-in partner Mr. X is

seventy years old. Her second to the youngest son is twenty years old, he has a live

in partner aged eighteen years old and have a son who is one year old. Her youngest

son is single with an age of seventeen years old.

Mr. & Mrs. X’s children visits them occasionally. The family bonding is still

there. Even though they have misunderstandings sometimes, they settle it it right

away for the sake of their family.

Mr. and Mrs. X and her two sons budgets their salary to meet their daily

expenses. Mrs. X and her partner earns P6000 per month by supplying water

spinach to the wet market. Her two sons earns P4000 per month by working on

construction site around Cabuyao. There is P10000 in total of income but is still

inadequate to provide enough food, shelter, electricity, and clothing for each

member. Mrs. X is the one who is responsible in decision making and budgeting the

money for the whole month.

All of the family members has finished only elementary level due to financial

problems. They are all Roman Catholic. Mrs. X is aware that there are organizations

that exist in their barangay. In fact, she is a member of some organizations such as

Butong Emergency Response Team (BERT), Red Cross, and Kababaihan. She also

participates in meetings regularly.

C. Home and Environment

The Family X’s house is made up of makeshift materials that include

woodcraft, old tarpaulins, old boxes and patchwork of different crafts. They just built

their house in the area that makes them an informal settler. There is no division

between kitchen, living room, and bedroom where the members tend to eat, rest, and

sleep. There is one bamboo bed that serves as their sleeping place wherein there are

some clothes that unfolded and unorganized. According to Mrs. X, it makes her

uncomfortable at night because of inadequate space to accommodate all of them also

they don't have an electric fan. For their safety against the area surrounding their

house is prone to flooding during rainy seasons and smelly water like burak, thus

mosquitoes and flies are present.

Their water supply comes from “poso” which is used by the entire

neighborhood. The water coming from “poso” is their supply for laundry, bathing,

and for their drinking purposes. The water that is intended for bathing and laundry is

placed in a covered container while the water that is used for drinking is stored in an

old plastic bottle. When it comes to their dish rack for eating utensils and plates it is

uncovered. And for their garbage disposal, they gather it then it is taken out every

other day for garbage collection.

When it comes to toilet facilities, they share a bathroom with one of Mrs. X’s

relatives because they don’t have their own. The neighborhood is congested, unsafe

and it is a bad environment for everyone because of improper waste management of

some neighbors, flood brought by the typhoon and pod woodcraft bridges to reach

their home. They have a television and radio for entertainment. They use public

transportation everyday. The health center is just a five minute drive from their home

which makes it easily accessible for them in case of emergencies and health

problems.

Mrs. X has a history of diabetes. She does not want to come back to the

doctor for further check-up for the reason of being scared to undergo dialysis.

Complication of her diabetes can be considred as risk factor for assessment. All of

them experiences common illnesses like cough and colds. Their drinking water

supply is a risk for gastrointestinal disease and their environment is prone to insect

causing disease. The one year old child has a history of diarrhea. Furthermore, he is

also not yet completely immunized but according to the mother, they are

coordinating with the health center for the immunization process.

For check ups and health problem, they preferably go to the health center to

have free consultation and medicine. They have lack of knowledge about drinking

unsafe water which may lead to other illnesses that may affect the health of their

family members.

E. Values, Habits, Practices on Health Promotion, Maintenance and Disease

Prevention

The adopted family is having a problem about health since there is and lack

of education with regards to health aspects. They encountered most common

illnesses like cough, colds, and diarrhea. For check ups and health problem, they

preferably go to the health center to have free consultation and medicine.

As a family they just have each other for problems other than health such as

financial problems. They do not have enough rest and sleep because of their work,

environment and inadequate space for all of them. For entertainment and relaxation

purposes, they watch television and listen to the radio. According to Mrs. X, their

relationship with the neighborhood is wholesome since they are generous and kind.

The nearest social and health facilities are the health center and the Office of the

Barangay. They are also aware that there are organizations in the barangay such as

Kababaihan Group and Butong Emergency Response Team (BERT). Mrs. X

participates in the given organizations above by attending meetings regularly.

CHAPTER IV

SPOT MAP

A. Narrative Description

The adopted family resides in Purok 1 Brgy. Butong Cabuyao, Laguna. Upon

reaching the arch going to Butong, there is a store located on the left side. That

will be the landmark of their house. Walk straight ahead on the left side of the

store, then until end of the alley is reach, that is the house of our adopted family.

The Butong Health Center is a 5 minute ride from their home. Every bridge

is an indication of each purok. Straight ahead, a basketball court will be seen that

is located at purok 2. Preceding at the main road, St. Joseph Village 4 is on the

right side while Butong Elementary School is on the left side which is Purok 3.

Continuing ahead, there is a bridge that indicates the Butong Health Center on the

left side of the national road.

CHAPTER VIII

EVALUATION AND RECOMMENDATION

EVALUATION

After 3 days of home visit, they showed some improvements on the said

problems. The student nurses oriented the family about the importance of cleanliness

in the house and discussed how to organize the house to provide more spaces for

them. After the nursing intervention, the family cleaned up and organized their

house.

The student nurses also oriented the family about the risk of drinking water

from a “poso” near stagnant water and lake, they stored water in an inappropriate

storage. The family still drank the water from “poso” but followed the instructions of

the student nurses on boiling the water before drinking. The family did not apply the

said insruction because of their beliefs and culture.

The student nurses educated the adopted family on how to properly protect

themselves from insects like flies and mosquitos, even though the family was using

mosquito net, it was not adequate to protect themselves at day time, the student

nurses suggested about the usage of insect repellant lotion or katol for the mosquitos

on day time and even at night. Usage of fly trap and insect spray for flies and to

cover their food everytime. After the nursing intervention, the family used katol for

the mosquitos and covered their food.

The student nurses clarified to the family on how to properly take care of

athlete’s foot, the student nurses suggested on go first to the center and apply some

alternative treatments. The student nurses also showed the proper way to wash their

feet with soap after they wade to the water and also suggests ways on how to keep

the rubber boots clean. After informing them, the family followed the instruction in

proper cleaning of their feet after wade on the water. And used rubber boots as a

protection. They also verbalized that “ itatry ko na yung mga maligamgam na tubig

na may bawang bago ko ulit gamitin yung krudo”.

The student nurses also explained what are the complications of Diabetes

Mellitus for Mrs. X so that she will be aware on what would happen if she would not

medicate and go to the hospital for treatment. The student nurses also imparted how

to have a lifestyle and proper food selection like avoiding too much sugar and having

a proper exercise because of her diabetes. After educating the family, Mrs. X

verbalized “sige, sa martes babalik na ako sa center para magpa check up at

manghingi ng request form para magpatest sa sugar ko.” On changing the lifestyle,

Mrs. X was not fully determined on changing hers, she verbalized that “ yun nga kasi

mahirap eh, lalo na mahilig ako sa kanin”.

RECOMMENDATION

● Adopted family should pay more attention to their health status. They should

apply the knowledge they acquired on the health teaching of the researchers

to their lifestyle.

● Community health workers and barangay officials should be more aware of

the conditions of every family in their community. If possible, visiting every

house in the community to offer help and services would be effective.

● Members of the community of Baranggay Butong should participate in

programs and projects regarding health to widen their knowledge to different

aspects of health that can benefit them in their healthy living.

● Government branches such as DOH should hold different programs and

orientations in different places to improve the quality of health conditions in

community.

● Future researchers will benefit from this study as a guide or paradigm to their

related studies.

● This study is still open for references and certain revisions.
