- If there are any changes to the pre-stated conditions, individual shall resubmit the form.

Please note that you are expected to undergo a 7-day self-quarantine at home before mobilization to
the EMCL site. Please answer the following questions truthfully. The following questions will help us
to understand your risk profile and potentially implement further mitigations or safeguards that
protects you and your co-workers. Intentionally providing wrong information in the form will be
treated as a breach of your contract with EMCL or a violation of ExxonMobil’s Fair Play governance.

melakukan mobilisasi ke area kerja EMCL. Mohon menjawab pertanyaan dengan sejujurnya. Kuesioner

tindakan pengamanan untuk melindungi anda dan rekan kerja anda. Pemberian informasi yang salah di
form ini secara sengaja akan dianggap sebagai pelanggaran terhadap kontrak dengan EMCL atau
diberlakukan penerapan prinsip Fair Play ExxonMobil.

☒ I consent to the collection and process of my data, as well as my household members’, for COVID19
potential transmission prevention including the disclosure of the same to the government authorities as
required. I consent for the full compliance of the protocols as part of a joint effort to maintain safety and
health. Non-compliance with protocols that may lead into an infection or cause impact on personnel safety of
the individual or co-workers health and sustainability of BU operations will be treated as a breach under our
contract with EMCL (i.e. the employment contract or the service contract, as the case maybe) or a violation
under the ExxonMobil’s Fair Play governance which may result in sanction imposed by EMCL under such
contract. Furthermore and for similar violation of compliance in case of contractor personal, EMCL reserves
the right to take consequences including discontinuation of making business with the contractor company
and/or contractor.

awal dari penerapan prinsip Fair Play oleh ExxonMobil yang dapat berakibat sangsi oleh EMCL dari
kontrak tersebut. Lebih lanjut dan untuk kasus yang sama mengenai ketidakpatuhan peraturan oleh pekerja
kontraktor, EMCL memiliki hak untuk melakukan beberapa tindakan yang diperlukan termasuk pemutusan
hubungan bisnis dengan perusahaan kontraktor dan/atau pekerja kontraktor.
