JOINT AFFIDAVIT OF TWO 2 DIS-INTERESTED PERSONS

We, JESSICA P. ORCULLO and ALONA C, SARNO, both of legal ages, Filipinos and
with addresses at # 348 General Emilio Aguinaldo Hiway Salitran I, Dasmarinas City, Cavite,
and # 066 Florence Village C. Aguinaldo St. Salitran I, Dasmarinas City, Cavite, respectively,
after having been duly sworn to in accordance with law hereby depose and say:

That we are friends and neighbors of CHANDICE BETIA MARI A. ROBLES, resident
of Blk 6 Lot 63, Illinois St. Ivory Crest Subd, Salitran 2, Dasmarinas City, Cavite;

That in her record of Live Birth Certificate, her name appears as CHANDICE BETIA
MARI A. ROBLES with birthdate on August 5, 2001; However, in the record of her
BAPTISMAL CERTIFICATE, her name is erroneously written as CHADEA BETIA
MARIE ROBLES and born on August 31, 2002, instead of the correct name CHANDICE
BETIA MARI ROBLES and correct birthdate August 5, 2001;

That the CHANDICE BETIA MARI A. ROBLES born on August 5, 2001 and CHADEA
BETIA MARIE ROBLES born on August 31, 2002 as described herein refers to one and
the same person;

That we are executing this affidavit to attest to the truth of the foregoing for all legal
intents and purposes.
