EMPLOYMENT CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Employment entered into this ___th day of _____ in the City of
Tagbilaran, Philippines by and between:

_________, Filipino, of legal age, married, and a resident of __________,


Philippines, hereinafter referred to as the EMPLOYER,

- and-

______________, Filipino, of legal age, married, and a resident of


_______________, hereinafter referred to as EMPLOYEE,

WITNESSETH:

WHEREAS, the EMPLOYER is the proprietor/manager of ___________, an


establishment engaged in the business of distribution and dealership of generic medicines
and pharmaceutical products in the Province of Bohol;

WHEREAS, it is to the best interest of the EMPLOYER to hire the services of a


_________ to undertake the duties of the said distribution and dealership of the generic
medicines and pharmaceutical products;

WHEREAS, the EMPLOYEE is capable of, and is willing to undertake the


services required by the employer;

NOW THEREFORE, for and in consideration of the above premises, the


EMPLOYER hereby retains the services of the EMPLOYEE, and the EMPLOYEE
covenants to undertake the responsibilities required of the job, subject to the following
employment terms and conditions:

1. This employment contract shall be for a period of __ (_) years beginning on


___up to ____. Any willful violation of any of the terms of this contract shall be
sufficient cause for the termination of this employment at any time within the six (6)
month probationary period. At the end of the six (6) month probationary period, and upon
showing of competence for the task assigned as determined by the EMPLOYER upon
recommendation of the EMPLOYEE’s direct superior, the EMPLOYEE shall be
accorded the permanent status. A separate employment contract shall then be signed by
the parties to this employment contract evidencing the change of the employment status;

2. The compensation of the EMPLOYEE shall be ____ (P000.00) PESOS per


month, representing the Basic Salary of said EMPLOYEE, which shall then be payable
every 15th and 30th of each month. It shall be understood that the policy of the employer
shall be “NO WORK, NO PAY”.

3. The EMPLOYEE shall exercise extraordinary care and diligence in discharge


of the duties assigned to said EMPLOYEE;

4. Upon proof of negligence on the part of the EMPLOYEE, said EMPLOYEE


shall be held liable for any and all damages and/or loss occurring by reason of said
negligence in the discharge of the functions of her office, which shall consequently result
to the termination of said EMPLOYEE;
IN WITNESS WHEREOF, the parties hereto have hereunto set their hands this
____ day of June, 1998, in the City of Tagbilaran, Philippines.

known to me and to me known to be the same persons who executed the foregoing instrument
and they acknowledged to me that the same is their free act and voluntary deed.

This instrument a CONTRACT OF EMPLOYMENT consisting of two (2) pages


including the page on which this acknowledgment had been written is signed by the parties and
their witnesses on each and every page thereof.

IN WITNESS WHEREOF, I have hereunto set my hand the day, year, and place first
