    Sir:

    We respectfully request approval for the release of our outstanding collectible with your Agency
    under Purchase Request PR# 15-07-666 dated July 07, 2015 for the various supplies and
    materials which were delivered to, duly inspected and accepted by the end user, the Pasig River
    Ferry System on [date] in the amount of Pesos: Three Hundred Seventy Four Thousand Two
    Hundred Ninety-Five (P374,295.00).

    This account was only discovered to be unpaid after we received a query from MMDA’s resident
    auditor, Mr. Nelson C. Talabucon, on January 21, 2020, requesting information from us, to
    which we confirm its existence after verifying our own records. We humbly ask consideration
    for our inadvertence on this matter as we honestly believed all transactions we had with MMDA
    were completely settled then. The voluminous paper works we simultaneously handled during
    the time the series of transactions were entered into must have caused this particular billing to be
    left unattended.

    We hope you find everything in order.
