AFFIDAVIT OF CORRECTION

I, SHERRY ANN CABALLERO CHIEN, of legal age, Filipino, married to JIA-SHENG

CHIEN, with residence and postal address at Block 07 Lot 49 The Cambria Subdivision, Sto.
Domingo, Bay, Laguna, after having duly sworn to in accordance with law do hereby depose
and state:

1. That, I was born on 16 February 1993, at Caramoan, Camarines Sur, to parents

Eddie Azaň a Caballero and Nida Basilio Ombao;

2. That, the name of my father was entered in all vital records of the family and his
proof of identity as EDDIE AZAŇ A CABALLERO; further, the same name was
appears in all identification cards;

3. That, however, his name was inadvertently entered in my Certificate of Marriage

as EDDIE ASAŇ A CABALLERO;

4. That, in view hereof, I hereby attest that the true and correct name of my father
is EDDIE AZAŇ A CABALLERO which information is evidenced by the said vital
records;

5. That, I am executing this affidavit to attest to the truth of foregoing facts, for
information and guidance of proper authorities concerned, for the correction of
entry as to the name of my father in my Certificate of Marriage and for whatever
legal purpose this affidavit may serve.
