This summary has been designed with your needs in mind. It provides easy access to the information you need about the optional
benefits available to you.
Group Benefits are important, not only for the financial assistance they provide, but for the security they provide for you and your family.

Manulife Benefits Basics


Your optional Benefits Program includes a range of services to help protect the health and security of you and your family. To highlight
our commitment to our standard contractors, the Company pays 50% of the monthly premiums for your single coverage. However you
may choose to add on your eligible dependents and the monthly premium will be divided by 4 and deducted off your weekly paycheque.
(Spouse: legal or common-law (12mths co-habitation); unmarried natural, adopted or stepchildren up to age 21 or 25 if enrolled in school).

PLEASE NOTE: This is an optional program. When Manulife refers to Default Coverage this means
No Coverage has been selected.
 Plan pays up to $100 toward eyeglasses or contact lenses every two years
EHC – Vision
 Visual training; maximum lifetime benefit $200
 Doctor Prescription: for sickness, injury, preventive vaccines, life-sustaining drugs, and oral contraceptives
EHC – Prescription
Drug  Plan pays 70%, you pay 30% for the mandatory generic substitution
- If a brand name is preferred, you will pay the difference
 Deductible equal to dispensing fee
 Plan pays 70%, you pay 30% for covered services
EHC – Professional
Services - Chiropractor, podiatrist, speech therapist and psychologist, osteopath, massage therapist, physiotherapist and
naturopath - $200/year for each service
 Maximum benefits of $400
Dental Care
- Basic Service: plan pays 70%, you pay 30%
periodontics, endodontics & denture reliners, rebasing & repairs
Health Service  Provides assistance in finding second opinions in your area

Costs: The employee weekly rates are listed below. The rates are based on the monthly premiums charged by Manulife, less
the employer portion and divided into 4 in order to be conveniently deducted off our contractors weekly pay cheque:

• In the event that a deduction has been missed it will be doubled deducted until the account is brought back up to date.
• Rates are in effect from October 1, 2013 to September 30, 2014 and rates are subject to change.

 Standard contractors can set up a weekly contribution into their RRSP account directly with Sun Life upon hire date

Retirement Savings  Save up to 18% of annual income (subject to CRA limits) each year on a pre-tax basis
Plan (RRSP)  To manage your accounts online or by phone, you’ll require a personal access ID and password. You’ll need an
account number, which you’ll find in the welcome letter you receive from Sun Life Financial, and your date of birth,
to register for these numbers: (refer to your welcome letter mailed to your home address for your account number)
• go to mysunlife.ca;
• select Register now; and follow the instructions provided.
• select my financial centre from my financial future on the Home page;
• select Enrol from the Requests drop-down menu and follow the instructions provided.

Other Benefits
Employee  Provider: Health Advocate
Assistance Program  Company paid confidential counseling for emotional, legal, financial, and other personal issues
(EAP)  Maximum 3 sessions per issue
Other Programs  Paid time off (refer to Employee Handbook)  Affinity programs such as Auto and Home insurance
