On <date> an inspection of the above noted project was carried out by (Consultant, Project
Sponsor and Contractor) and, with the exception of <Footnote 1>, all work on this contract has
been satisfactorily completed.

You are therefore advised that this contract is conditionally accepted subject to the following:

1. The Department will hold an amount equal to $ <Footnote 2> to cover the cost of correcting
these deficiencies.

2. The repair of deficiencies is to be completed by <Footnote 3>. If the repairs are not completed by
this date the Department reserves the right to initiate action to have the work completed utilizing
Contract Securities as outlined in the General Specifications.

Should you complete the deficiencies by <Footnote 3> the warranty period shall be considered to have
commenced on <Footnote 4>. If the repair of the deficiencies is not completed by <Footnote 3> then
the warranty period shall commence on the date that the repair of the deficiencies is actually completed.

You will be hearing from the Department’s Strategic Procurement Branch about providing the necessary
Statutory Declaration forms as required by the contract General Specifications

1. Either list deficiencies or refer to a letter dated “MM/DD/YYYY” which includes a list of deficiencies.
2. Typically, twice the estimated cost to complete the repairs.
3. Reasonable deadline to get deficiencies corrected (Usually June 15th of following year)
4. Use the date of the Conditional Construction Completion Inspection.
