AFFIDAVIT OF LOSS

I, JOSEPH E. CAÑETE, of legal age, Filipino citizen, and a resident of

Burias, Glan, Sarangani Province, under oath depose and state that:

1. I am the holder of a CSS NCII CARS NO. CSS171280131001451 and

CSS17128015100695, duly issued by the office of Holy Trinity
College, General Santos City;

2. However, the above-mentioned CSS NCII CARS NO.

CSS171280131001451 and CSS17128015100695 was lost in my
possession Last November 30, 2018 and despite diligent search and
effort exerted to locate or recover it, the same proved futile and in vain
such that I may now consider lost and beyond recovery;

3. Said CSS NCII CARS NO. CSS171280131001451 and

CSS17128015100695 was not confiscated by any authorities for
violation of any of its rules and regulations and was not given
assigned or pledged to any person or entity for consideration;

4. I am executing this affidavit to attest to the truth of the foregoing

facts ad for whatever legal purpose it may serve best;
