AFFIDAVIT OF EXPLANATION

I, FRANCISCO T. DELA DRUZ, Filipino, of legal age, single, and a

resident of __________________________, after having duly sworn in
accordance with law, hereby depose and state that:

1. I was born biologically male and baptized in the Archdiocese of

__________________________ with the name of FRANCISCO L.
TAPIAN;

2. My name in my Certificate of Live Birth issued by the Philippine

Statistics Authority erroneously entered my first name as FRANCISCA,
which is a clear typographical error;

3. Due to the confusion caused by the erroneous spelling of my first

name, my sex in the same Certificate of Live Birth was also
erroneously entered as “FEMALE” instead of “MALE”, when in fact, I
am born biologically male;

4. Hence, I now want to correct in my Certificate of Live Birth the

aforementioned entries which are clearly clerical and typographical in
nature;

5. I execute this affidavit to attest to the truth of the foregoing facts and
for whatever legal purpose it may serve.
