Lewis County Announces Vaccine Distribution Plan


January 11, 2021

Lewis County Public Health will be setting up three COVID-19 vaccine clinics for the week of
January 11th – 16th. At this time, vaccine registration is open ONLY to individuals who qualify in
Phase 1A and Phase 1B of the New York State Vaccine Distribution Plan.

A list of eligible individuals is attached to this release, and can be found on the State’s COVID-
19 Website. Only those individuals who live or work in Lewis County are eligible to attend this
week’s clinics.

Clinics will be held at the following locations:


• Tuesday 11/12 at the Lowville Elks Club from 1:30 to 5:30 P.M.
• Wednesday 11/13 at the Constableville Fire Hall from 1:30 to 5:30 P.M.
• Thursday 11/14 at the Croghan Fire Hall from 1:30 to 5:30 P.M.

Eligible individuals can register online at www.lewiscounty.org/vaccine, or simply visit the


County website homepage and click on the COVID-19 Vaccine banner. Those without internet
access and those ages 75 and older can call the County’s vaccine registration hotline,
(315) 376-9777. A County employee will register you for an appointment.

Eligible individuals with reliable internet access are encouraged register online, freeing up phone
lines for the older individuals and those without internet access.

Please note that proof of eligibility will be required at the time of your appointment. Those who
fail to register for an appointment and provide proof of eligibility will be turned away at the
door.

Proof of eligibility includes workplace ID, workplace pay stub, or written proof of employment
from your employer. For those 75+ year-olds, an ID containing date of birth will suffice.

Eligible New Yorkers in Phase 1a and 1b are:


• High-risk hospital workers (emergency room workers, ICU staff and Pulmonary
Department staff)
• Residents and staff at nursing homes and other congregate care facilities
• Federally Qualified Health Center employees
• EMS workers
• Coroners, medical examiners and certain funeral workers
• Staff and residents at OPWDD, OMH and OASAS facilities
• Urgent Care providers
• Individuals administering COVID-19 vaccines, including local health department staff
• All Outpatient/Ambulatory front-line, high-risk health care workers of any age who
provide direct in-person patient care
• All staff who are in direct contact with patients (i.e., intake staff)
• All front-line, high-risk public health workers who have direct contact with patients,
including those conducting COVID-19 tests, handling COVID-19 specimens and
COVID-19 vaccinations
• This includes, but is not limited to,
o Doctors who work in private medical practices and their staff
o Doctors who work in hospital-affiliated medical practices and their staff
o Doctors who work in public health clinics and their staff
o Registered Nurses
o Specialty medical practices of all types
o Dentists and Orthodontists and their staff
o Psychiatrists and Psychologists and their staff
o Physical Therapists and their staff
o Optometrists and their staff
o Pharmacists and Pharmacy Aides
o Home care workers
o Hospice workers
• Staff of nursing homes/skilled nursing facilities who did not receive COVID vaccination
through the Pharmacy Partnership for Long-Term Care Program

o Public Safety Communications
▪ Emergency Communication and Public Safety Answering Point Personnel,
including dispatchers and technicians
o Other Sworn and Civilian Personnel
▪ Court Officers
▪ Other Police or Peace Officers
▪ Support of Civilian Staff for Any of the above services, agencies, or
