Her first born is a term 2-year-


old female. Negative uterine contraction, negative vaginal discharge. No history of hypertension. Patient
is admitted at Day 1, 4:41PM

Admitting diagnosis: G3P2, pregnancy uterine (PU), 37 weeks 5 days AOG, Prev CS II

Day 1

 Secure consent for admission
 Secure consent for the procedure
 Schedule contemplated procedure at Day 2, 8:00AM
 Inform operating room
 Notify Anesthesiologist on Duty
 Dx: CBC, urinalysis, BT crossmatching, Clotting time, bleeding time
 Secure Blood
 IVF: PNSS 1L x 12 hours prior to OR
 Cefuroxime 1500mg IV ANST prior to OR.
 Monitor
 Refer

 Skin test ketorolac prior to OR


 Skin test lidocaine
 NPO post-midnight
 Complete bed bath
 Gargle with hexetidine

 Ward Nurse will endorse to operating room Nurse


 Accomplish pre-operative checklist
 Transfer of patient from ward to operating room
 Position the patient for induction of anesthesia
 Position the patient for the procedure
 Principles of sterility
 Surgical Scrubbing
 Steps on how to do gowning
 Steps on how to do gloving (either open and closed)
 Familiarity of instruments

Day 3 Post-operative

11:12AM Doctor’s order

 Transfer patient to PACU


 Flat on bed for 6-8 hours
 NPO until further orders
 Oxygen support at 2lpm
 Monitor VS with O2Sat for 15 minutes then hourly until stable
 Monitor I & O accurately Q1
 Cefuroxime 750 mg IV Q8
 Ketorolac 10mg IV Q6 x 24 hours
 Tramadol 50mg IV PRN
 IVF: D5LR 1L x 8 hours to alternate with
 PNSS 1L 1L x 8 hours
 Monitor
 Refer
