    I, Rench Gerald C. Loreto, legal age, residing at San Juan, Apalit


    Pampanga, currently enrolled as an Engineering student, with student
    number 2015-152196 in Don Honorio Ventura State University, am
    writing this letter to notify you of my intent to allow the College of
    Engineering and Architecture to enroll me as a first year student to catch up
    for my back subject as well as to be updated to the new curriculum.

    I allow it because there is no available instructor/professor that can teach


    those subjects that I failed in a special class manner.
