AFFIDAVIT

I, CLARITA MAAÑO CUAREZ, of legal age, married, Filipino citizen,

and resident of 35 City Camp Proper, Baguio City, Philippines, do hereby depose
and state that:

1) I am the father of one MARY ANNE CLAUDITH ABAYON

CUAREZ, who was born on 11 December 1996 at Quezon Memorial
Hospital, Lucena City, as indicated on her Certificate of Live Birth, attached
and marked hereto as Annex “A”;

2) The name of her mother, as appearing on her Certificate of Live Birth,

is Juditha Abayon Cuarez, which is already her married name instead of
her maiden name;

3) The maiden name of her mother is actually Juditha Brajasa Abayon,

as seen in our Certificate of Marriage, attached and marked hereto as Annex
“B”;

4) I am currently processing the release of my daughter’s Philippine

Passport from the Department of Foreign Affairs;

5) I am executing this Affidavit to attest to the truth of the foregoing, to

satisfy the requirements of the proper government agency, and for all legal
intents and purposes that it may serve.

WHEREFORE, I have hereunto set my hand this 25th day of April 2016, in
the City of Baguio, Philippines.
