AFFIDAVIT

I, KATHRINA VANESSA Y. HAGORILES, Filipino citizen, of

legal age and a resident of Brgy. Silway 8, Polomolok, South
Cotabato, after being sworn to in accordance with law hereby
depose and state:

1. I am a registered taxpayer with TIN

2. My real name, as indicated in my Certificate of Live Birth

issued by the National Statistics Office (Attached herein as
ANNEX “A”) and in my Baptismal Certificate issued by St.
Julian de Cuenea Parish of Janiuay, Iloilo (Attached herein
as ANNEX “B”) is RONALDO BORRA GABARRA.

3. I have registered under the name REYNALDO BORRA

GABARRA because when I was in elementary, my
teachers began using the said name in my records to refer
to me, and have called me using the said name. Being
young and unaware that it would cause a discrepancy to
my records in the future, I eventually accepted and used
“REYNALDO” instead of “RONALDO”.

4. The said discrepancy constrained me to execute an

affidavit attesting that REYNALDO BORRA GABARRA
and RONALDO GABARRA refer to one and the same
person.

5. Lest there might be future discrepancies as regards to my

name, I henceforth desire to change the same and cause
its correction.

6. I further execute this instrument to attest to the truth of the

foregoing facts and for whatever legal purposes it may
serve.

REYNALDO BORRA GABARRA refer to one and the same
person, and that we are neither related by affinity nor
consanguinity.

3. We execute this affidavit without offered by any reward or

recompense and further executing voluntarily execute this
Affidavit supporting all the averments and the above-
enumerated statements made by RONALDO BORRA
GABARRA as true and fact according to our knowledge.
