Student Stress Questionnaire
Please read the information sheet and, if you are happy to take part in the study, please
sign the consent form attached before starting this questionnaire. As stated in the
information sheet, this study is completely anonymous and all data will be kept
confidential. The data will only be used for the purposes of education (coursework) and
will be destroyed upon completion of the work.

Study Title

An investigation into stress, anxiety and depression in students at a university in South Wales,

UK

Invitation paragraph

I invite you to engage in a research project to analyze students' reactions to the use of alcohol

at a South Wales University. Before you vote, you must understand about the research which is

on-going and what it means to you. Please take time to read the description attentively. If

something you read isn't easy or you want more information, ask questions. Take the time to

decide whether you want to enter or not.

The thesis would explore the attitudes of students at a South Wales University towards alcohol

use. By analyzing the data we gather from this investigation, we will able to conclude how

students at a South Wales University are concerning alcohol use.

What is the purpose of the study?

The aim of this analysis is to see the attitude of students towards alcohol and it’s link with how

much alcohol he/she is drinking. We can also be able to see some trends between different

races, classes and age groups with their alcohol intake and their attitude toward alcohol. We

could conclude by looking at whether or not an optimistic attitude on alcohol helps to improve

alcohol consumption.
Why have I been invited?

A total of 100 people will be participating in this research. University participants are drawn from

South Wales, UK. Students from the 1st, 2nd, and 3rd years and also post graduate students

will participate in this research. There will also be a diversity of races and ages, so we can

compare the different factors to figure out what their attitude is towards alcohol.

Do I have to take part?

It is totally up to you whether you want to participate with the study or not. We can describe the

study in detail and will help you to determine whether you would like to participate in this

research. If you wish to join, we will ask you to sign a consent form. You can withdraw from the

analysis at any point.

What will happen to me if I take part?

You are lead to a private space to complete a questionnaire if you wish to participate in the

study. The questionnaire is finished in 30 minutes and all your data will keep private.

Expenses and payments

You are at the university during this research survey, meaning that your lectures, commute and

work hours are not disturbed, so no payments will be given to you.

What will I have to do?

All you have to do is to complete the questionnaire which will take only half an hour and then

pass it to the investigator. You are already in a University premises while the research takes

place, so it is convenient for you.

What are the possible disadvantages and risks of taking part?

You may have potential risks when you fill out a questionnaire. You may notice about yourself

how much alcohol you consume, you may become nervous or worry about, so the necessary
resources will be there to assist you.

What are the possible benefits of taking part?

We don't give assurance that this research will benefit you, but the knowledge we get from this

study allow to strengthen the understanding of the student's approach to the use of alcohol.

What if there is a problem?

You may invite the researcher to talk to you if you have any questions on any part of this report.

You should reach the researchers by e-mailing them:

Will my taking part in the study be kept confidential?

The data gather about you throughout the survey will be kept purely private and you will not

have to give personal information at any point during the survey.

Involvement of the General Practitioner/Family Doctor (GP)

Your general physician will not be called for this survey, because during the process of the

research no harm can come to you.

What will happen if I don’t carry on with the study?

You can withdraw at any moment from the survey and all the knowledge and information

gathered from you will be deleted.

What will happen to the results of the research study?

The study findings won't be reported and posted for the people to see. The data can only be

accessed by the researcher and then deleted after 5 years.
