Subject: DBNMPC CDO Extension Office BIR Registration

Dear Dir. Dela Cruz:

Greeting.

We most humbly request for the examination of the attached documents related to the DBNMPC CDO
Extension Office BIR Registration and for the approval and release of the said BIR Registration.

We had applied for BIR registration on May 20, 2008 and started our CDO operation on June 2, 2008.
Since then, the CDO Extension Office used an accounting system but only until July 1, 2016 when
DBNMPC centralized the entire coop accounting and bookkeeping. It is now the main office of DBNMPC in
Balamban, Cebu that does the recording of the transactions including those of the CDO Extension Office.
The CDO Extension Office just maintains a petty cash fund for its immediate and minor expenses and
sends the petty cash fund liquidation to the Balamban, Cebu office for replenishment.

Formerly the DBNMPC office address was at the Salvacion Bldg. Zone 2 Bugo, Cagayan de Oro City.
Since July 1, 2016, DBNMPC has transferred and now holds office at the 2nd Floor, Lopez Bldg., San
Pedro Gusa, Cagayan de Oro City.
