SIMPLEX HEALTH CASE MANAGEMENT PLAN —
Our medically supervised professional staff apply transformational evidence-based protocols and therapies
treating the person, not the symptom. Through our team of Registered Dietitians, Health Coaches and Physi-
cian Advisors, the Simplex Care team works alongside the employee’s doctor. Through our HIPAA-secure por-
tal, mobile app, and virtual or in-person appointments, the care team will guide employees through Simplex
Health’s evidence-based protocols most suitable for each individual’s biology and lifestyle. Our HIPAA secure
platform provides a whole person approach for patients with access to Health Coaches, recipes, exercise sup-
port, and more (all included with our Wellness Plans). We work in partnership with the employee’s physician to
create a 360-degree experience for both physicians and patients that includes scheduling, billing (if applica-
ble), communication, and reporting patient outcomes.

DIABETES CASE MANAGEMENT PLAN

Simplex Health’s Diabetes Case Management Plan includes our full care team, plus a Certified Diabetes
Educator (CDE) oversight that helps employees with insulin management providing evidence based pro-
tocols and education that can reverse Type 2 Diabetes and manage Gestational Diabetes. Our program
trains and provides glucose meters, test strips and lancets, and utilizes remote monitoring of glucose
logs to deliver to employees’ physicians for newly diagnosed diabetics. Simplex Health patients have
successfully reversed prediabetes, diabetes, obesity, and other symptoms of metabolic disease with
77% of Simplex Health patients with Type 2 Diabetes have reduced their A1c levels.
Our Gestational Diabetes Management Program, part of the Diabetes Case Management Plan, has suc-
cessfully kept 88% of our patient population off of medication and maintained their health with dietary
intervention and monitored care. Employees can receive glucose meter training, supplies, and weekly
one-on-one nutrition education appointments. Meticulous chart notes are faxed to the employee’s phy-
sician at the close of each appointment, and weekly glucose logs are sent to the employee’s physician.
Our Clinical Director of Diabetes and Prevention monitors all glucose logs for this high risk population
and immediately notifies the patient’s physician if there are consistently high glucose levels.
Diabetes Care Management Plan Case Study: Charlie, a 38-year-old male, lowered his glucose
from 266 to 84 and his A1c from 10.8% to 5.4% by following Simplex Health’s Simplex15 Protocol.
