Government's rules and regulations.

Appendix A

This permission is granted subjected to the following conditions prior to entering Malaysia:

i. The applicant is ENCOURAGED to undergo COVID-19 test screening abroad within THREE (3)
days and medically confirmed to be tested negative from COVID-19 prior to entry to Malaysia. The
test result is to be shown to the medical team at the departure point.
ii. The applicant is COMPULSORY to download the MySejahtera mobile application on their
smartphone and complete all the necessary information; and
iii. The applicant who is currently abroad is required to obtain a visa from the respective Malaysian
Embassy/ Consulate General/ High Commission prior to entry to Malaysia. Visa application is only
applicable for nationalities that require a visa to enter Malaysia.

The applicant is required to provide the following information upon entry and must comply with the
following conditions upon arrival:

i. The COVID-19 test result from abroad is to be shown to the Ministry of Health’s Medical Officer at
the entry point (if any).
ii. The applicant will be required to undergo another COVID-19 test by the Ministry of Health (MOH).
iii. To provide to the Immigration Officer at the entry point
a. Entry Approval Letter
b. Valid Pass / Approval Letter / Visa With Reference (VDR)
c. PCR Covid-19 Test Abroad Result (if any)
iv. The applicant is COMPULSORY to undergo QUARANTINE for SEVEN (7) days with a PCR Covid-19
test abroad result and TEN (10) days without a PCR Covid-19 test abroad result at a Quarantine
Centre stipulated by the National Disaster Management Agency (NADMA) and Ministry of Health
(MOH).
v. All cost (i.e.: COVID-19 test, expenses during quarantine, etc) upon arrival into Malaysia shall be
borne by the applicant.
vi. Failing to pay ALL cost due to the Government of Malaysia, the applicant’s active pass will be
cancelled, blacklisted in Immigration system and issued with Notice of ‘Not To Land’ (NTL) for
deportation.
vii. Adhere to procedures of entering Malaysia and existing Immigration regulations

Following the declaration of COVID-19 as a path by the World Health Organization (WHO), you are
under the Control of Prevention and Control of Infectious Diseases Act 1988 and the Police Act 1967.
Any person who disobeys any lawful order, directions or restrictions imposed by any authorized officer
or has breached any rules or regulations made under the Control of Prevention and Control of
Infectious Diseases Act 1988 commits an offence under of the said act.

Please be aware that any false reports/statements/representations are an offence under the Section
56(1)(f) Immigration Act 1959/63, and is punishable with a fine amounting to no more than
RM10,000.00, or imprisonment of no more than 5 years, or both.
