I Robert Jess Baluyut, of legal age, 24 years old, Filipino and residing at Sta.
Cruz Macabebe, Pampanga, after having been duly sworn to in accordance with
the law, do hereby depose and state that:

1. All documents that I have submitted in relation to my application for an

Administrative Assistant position in the Department of Education, Division
of Pampanga are authentic and genuine.

2. I am giving full authority to the Department of Education, Division of

Pampanga to verify the authenticity/veracity of the said documents.
