This is to certify that TRAVELLERS INSURANCE & SURETY CORPORATION is licensed

to transact non-life insurance business in the Philippines for FIRE, MARINE, CASUALTY,
SURETY lines under Certificate of Authority No. 2019/17-R effective 1 January 2019 until 31
December 2021, unless sooner revoked or suspended for cause.

It is certified, moreover, that TRAVELLERS INSURANCE & SURETY CORPORATION is

authorized under its license to issue Contractor’s All Risk Insurance policies and that the
insurance company had issued a CONTRACTOR’S ALL RISK POLICY with Policy No.
CAR-41004 to the insured DJCM CONSTRUCTION CORPORATION in the amount of TWENTY-
ONE MILLION FIVE HUNDRED SEVENTEEN THOUSAND EIGHT HUNDRED THIRTY-NINE
PESOS AND FORTY-THREE CENTAVOS ONLY (Php 21,517,839.43) for the project:
CONTRACT ID NO. 20A00227 - TULAY NG PANGULO PARA SA KAUNLARANG PANG-
AGRARYO (TPKP) PROJECT: 1.) CONST. OF KAKALDINGAN BRIDGE, BRGY. LUNGOG,
NARVACAN, ILOCOS SUR, 2.) CONST. OF SILAG BRIDGE, BRGY. SILAG, STA. MARIA,
ILOCOS SUR, AND 3.) CONST. OF NAGLAOA-AN BRIDGE, BRGY. NAGLAOA-AN, STO.
DOMINGO, ILOCOS SUR. Certified photocopy [or duplicate copy] of said policy was submitted by
the company to the Insurance Commission.

This Certification is issued upon the request of Mr. Marianito G. Faral, SVP - Comptroller
