AFFIDAVIT OF CHANGE OF COLOR

I, NANCY TE TAN-SANTIAGO, of legal age, and with address at c/o

R. Castillo St., Agdao, Davao City, after having been duly sworn
according to law, hereby depose and say that:

1. I am the Corporate Secretary of Davao Solar Best Corporation with

business at R. Castillo St., Agdao, Davao City;

2. The corporation is the registered owner of a certain motor

vehicle that is more particularly described as follows:

Make: Isuzu Motor No.: 6WA1-111642

Type: Truck Chassis No.: CYM50V2-3000448

Color: Yellow Plate No.: LXB-401

3. The original color of the said motor vehicle was "yellow";

4. When the paint of the above-described motor vehicle had

faded, we had its color repainted to "white" such that the color of the
said motor vehicle was changed from "yellow" to "WHITE";

5. Said change of color is not intended for any illegal or

unlawful purpose but solely due to the foregoing reasons;

6. I am executing this Affidavit to attest to the truth of the

foregoing and in support of the application for the change of color of
the above-described motor vehicle on its Certificate of Registration.
