(Address), declared for taxation purposes under the name – (Current Claimant) with ARP No.

That said property along with its adjacent lots were the subject of Civil Case No.___ entitled
Name of the Plaintiffs and the Defendants tried before under the Municipal Trial Court of
(Place of MTC) In its Joint Decision dated ________________, the Court dismissed the case in
our favor and same already attained finality. (Copy of the said Joint Decision and Finality is
hereto attached) In connection thereto, and as long time and actual occupants of Lot
NO.______, I am humbly requesting that the Tax Declaration under the name of
__________________ with ARP No. GR-________________________________________ be
CANCELLED and be TRANSFERRED under the name of (Name).

Attached also for your reference is the Certification from the DENR stating that the subject
property is covered by Miscellaneous Sales Application No. 051608-16 although no patent has
been issued and the process was “HOLD” due to the opposition of plaintiffs in the above-
mentioned case.
