Confirmation of Medical Cover

This letter provides a high-level summary of your cover. Full details of the benefits,
limitations and exclusions of this health plan can be found in the table of benefits in the
Membership Guide.

This is to confirm that Mr. Richard Llewelyn BI-6000-0204-9035 is a member of a Bupa

Global scheme. His policy provides benefit for the cost of hospital accommodation and
active treatment in line with our table of benefits up to an overall maximum of USD
2,890,000.00 per membership year,
(sub-limits exist for some specific benefits).

This policy also has a benefit for medical evacuation. We will only pay for Evacuation to
the nearest place where the required treatment is available when the required treatment is
not available locally. This could be to another part of the country that the member is in and
may not be their home country.

This policy also has a benefit for medical repatriation. We will pay for repatriation to the
members specified country of nationality or their specified country of residence, when the
required treatment is not available locally.

We can confirm that the cover is valid for all eligible treatment received Worldwide
excluding the USA

Cover is based on an annual contract that is due for renewal on 11/09/2021

During the COVID-19 pandemic Bupa Global has been providing cover for COVID-19
claims in line with individual policy terms, benefits and limits, relating to the diagnosis and
treatment, including treatment costs of hospitalisation and medicines related to COVID-19,
regardless of the epidemic or pandemic exclusion that may appear in your policy terms
