Please explain in writing within Five (5) Calendar days from receipt hereof why no disciplinary action, including dismissal,
should be taken against you due to the allegation/s below:
As of date, you have incurred 11.25 attendance occurrence points which is more than the maximum allowable attendance
points of the Company. This is despite the progressive advisories communicated to you by your Immediate Superior as listed
below:
Labor Code of the Philippines Article 297 (In Reference to Art 282): Termination by employer. An employer may
terminate an employment for any of the following causes:
a. Serious misconduct or willful disobedience by the employee of the lawful orders of his employer or representative in
connection with his work; and
b. Gross and habitual neglect by the employee of his duties.

• Failure to follow proper Call-In procedures for calling off of work. Employees are generally required to contact their
department supervisor directly, not through voicemail, if calling off work or if expecting to be late for work. (Refer to
the Attendance policy and location specific Call-In procedures.)
• Failure to comply with Company procedures, policies, memos, and/or instructions.
Attendance Policy
Under our Company’s Attendance Policy that was released on February 2018, regular attendance and punctuality
are essential to the successful operation of the Company. All employees are required to be at their workstations at
their scheduled start time so that they could promptly start working at the beginning of their assigned shift. Excessive
absences and tardiness, as described in our Attendance Policy, will result in disciplinary action up to and including
termination of employment, with due process.
Moreover, you are aware that employees who are unable to report for work at their scheduled work time must follow
the appropriate notification procedure of your site and notify at least 1 hour prior to the start time of their shift. Any
attendance infraction not adhering to the Call-In procedure shall automatically merit an additional Failure to Notify
(FTN) sanction.
Further, to give you additional opportunity to explain your side, you are also being notified and required to attend an
Administrative Conference with Human Resources and Operations on November 14, 2019 at 2:00AM (Manila) and proceed
to the Human Resources Office at the Alorica Davao 1 and look for Marinel Alladin. Kindly bring pertinent documents that
would further explain and present evidence in your favor in defense to the allegations set forth in this document.
Your failure to submit your written response within the required allowable time and/or attend the administrative conference as
scheduled will be deemed a waiver of your right to present your side and the appropriate disciplinary action will be imposed.
