Social Worker : “ Miss Rothschild, hi nice to meet you I’m Karen yin and
I’m the social worker here. Before you go in to see the
doctor, we have a few questions that we started. Asking
patients. I have them right here in front of you so these are
questions we want ask to find out more about who our
patients are what needs do they have how can we provide
quality care does that sound okay to you sure that my
appointments in 10 minutes is there time um oh yes it’ll
only take few minutes just want to point out that for any of
the questions you can see prefer not to answer and I do
want to let you know that the only people who can see this
in your file are the people who take care of you while
you’re in our care. “

Patient : “ okay.”

Social Worker : “ Otherwise it’s used for research and nothing can be
linked back to you personally.”

Patient : “ okay.”

Social Worker : “ okay, so the first questions is what language would you
feel most comfortable speaking in with your health care
provider.”

Patient : “ English.”

Social Woerker : “ okay which of the following best describe your a racial
or ethnic group.”

Patient : “ a white European.”

Social Worker : “ it’s your Man okay thank you um were you born in
Canada?”

Patient : “ No.”
Social Worker : “ okay so what’s your did you arrive here.”

Patient : “ at 72 73

Social Worker : “ okay an approximate this once output 73 um so what

your religius or spiritual affiliation?”

Patient : “ I’m Jewish.”

Social Worker : “ Jewish okay so on the next page you can flip that do you
have any of the following disabilities.”

Patient : “ I have diabetes is that.”

Social Worker : “ you want to put that down as a chronic illnes maybe.”

Patient : “ I usually okay.”

Social Worker : “ what does your gender? ”

Patient : “ female.”

Social Worker : “ okay and what’s your sexual orientation?”

Patient : “ I’m normal.”

Social Worker : “ all of them are normal diet so what we really want to
know is why you attrackted so did you mean to say may be
hetero sexual so female male.”

Patient : “ yes”

Social Worker : “ yeah okay thank you what’s your total family income
before taxes last year.”

Patient : “ an number two thirty thousand to 60,000.”

Social Worker : “ okay ... umhow many people does this income support

Patient : “2”

Social Worker : “ Ok just a few morequestions.”

Patient : “ and I will answer.”

Social Worker : “ and what language would you prefer to read your health
care information.”

Patient : “ English”

Social Worker : “English um and what type of housing do you live in.”

Patient : “ I’m not.”

Social Worker : “ so again we have list it can be owned and anything.”

Patient : “ I we all know no okay.”

Social Worker : “ okay and that’s it so you can just go and have a seat until
thank you very much. Okay thank you.”

Patient : “ thank you”
