    Description
    As per mentioned date, time & location, carry out unsafe lifting with boom truck & used improper
    lifting gear chain sling instead of web sling & D Shackle for lifting of sewerage pipes, also lift two
    pipes together (approximately 15 inch diameter). During lifting was observed that lifting radios
    area without barrier & out rigger pad was not place.
