Communication:
What communication systems will be used between the suspended worker and supervisor / rescue team?

Emergency Contact:
In the event of a fall from height, the supervisor will immediately alert the rescue team and first aid.
If the rescue team cannot affect a rescue within 5 minutes Emergency Services are to be called at once.

The structure is accessible from the exterior of the building from the 185’JLG but this means will not be
be the means to access from the bowl side. A crane basket shall be the primary means for rescue from
interior of the structure.(Where is the crane basket and who will be the operater on day or night if your
working two shifts?)/(If the crane is not available then what is the alternative?)

A determination has been made there are three different means for rescue. Primary shall be the aerial

There shall be no employee working alone at heights where this plan is required.

There is a difference in height from the exterior and the interior and it will dictate the means and methods for rescue.

Weather Conditions (Detail):


During summer months there can be extreme heat that can play a part in rescue activities. There are no other
“normal” weather conditions that pose any problem except wind storms or rain or thunderstorms.

Proximity to emergency services / hospital (Detail):


There are two hospitals less than 5 miles from the site. TriStar Summit Medical Center and TriStar Southern Hills
Medical Center are located within 10 minutes of the site for prompt medical care. If the employee is ambulatory, they shall be taken
to the nearest site authorized occupational health clinic specified by the OCIP/CCIP.

Language barriers (agency / contract staff) (Detail):


DIC has various employee who can translate for any Spanish only speaking employees if needed. At present this
Isn’t an issue.
