CONSENT FORM AND UNDERTAKING FROM PARENTS/GUARDIANS FOR SENDING
THEIR SON/DAUGHTER TO ATTEND PHYSICAL CLASSES FROM 01-02-2021

1. As per Memo.No.5640/SE.Prog.II/A1/2020 Govt. of Telangana dated 12.01.2021 for starting

of physical classes for professional colleges from 01-02-2021, the college has made the
following provisions to comply with the Standard Operating Procedures (SOPs) for Covid-19:

FACILITIES PROVIDED AND NORMS TO BE FOLLOWED IN THE COLLEGE AS PER

COVID-19 GUIDELINES

a. All the buildings of the college consisting of class rooms, laboratories, offices, library, and
corridors have already been sanitized in accordance with the prescribed norms and will be
periodically sanitized in the future also.
b. Not more than 50% of the class room capacity will be permitted to attend physical classes at
any given time, with one student per bench.
c. Physical distancing will be maintained in the class rooms, laboratories, corridors and open
areas.
d. All the students, faculty and staff are mandated to wear face mask. Students without a face
mask will not be permitted to attend classes as a precautionary measure.
e. Foot operated and hand operated sanitizer dispensers have been installed at appropriate
locations in the buildings.
f. Soaps will be provided in the toilets for washing of hands.
g. Students exhibiting Covid-19 symptoms will not be allowed to attend the classes.
h. Except for students, faculty and administrative staff, no other gathering will be allowed in
the campus.

CONSENT AND UNDERTAKING

2. I, Mr./Ms. ___________________________________________, Parent / guardian of

Mr./Ms. ___________________________, a student of ________________________,
bearing Roll No. __________________________ have been acquainted with the facilities
provided by the college for conducting physical classes from February 1 st, 2021.
 3. I declare that my son/daughter/ward will always wear mask, maintain social distancing,
wash hands or sanitize frequently.

4. I understand that the health and wellbeing of the community is the first priority, therefore
the college reserves the right to deny the entry to my ward in the premises.

5. I understand and acknowledge the contagious nature of COVID-19 virus and the risk of
exposure that may result from coming in contact with the infected person within or outside
the college.

6. I shall ensure that my son/daughter/ward carries his/her own food, drinking water and
sanitizer.

Having acquainted myself with the Covid-19 guidelines and the facilities provided by the college, I
am satisfied with the arrangements made and hereby give my consent to permit my
son/daughter/ward to attend physical classes from 1st February, 2021.
