Checklist For Antenatal Assessment

PRELIMINARY PREPERATION:
1. Prepare client area, necessary supplies and equipment.
2. Wash hands thoroughly with soap and water and dry with clean
dry cloth.
4. Tell the woman what you are going to do, encourage her to ask
questions and listen to what she has to say.

Weight: Weight should be taken in all cases
BREAST EXAMINATION:
ABDOMINAL EXAMINATION:
1. Ask woman to lie on her back with knees slightly bent and
uncover her abdomen.
OBSTETRIC GRIPS:
1. FUNDAL GRIP: Palpation is done facing the patient’s face.
Whole of fundal area is palpated using both laid flat on it to find out
which pole of foetus is lying in the fundus.
After completing the fundal grip, slip/ slide your hands to both lateral
sides of abdomen.( note- do not lift your hands while performing these
grips.)
While the hands are to placed flat on either side of umbilicus, press on
one side and palpate the other side to find out the position of the back,
limbs and anterior shoulder.
The oversteched thumb and four fingers of right hand are placed over
the lower pole of uterus keeping the ulnar border of palm on upper
border of symphysis pubis.
Four fingers of both hands are placed on either side of midline in lower
pole of uterus and parallel to inguinal ligament.
The fingers are pressed downward and backward in a manner of
approximation of finger tips to palpate the part occupying the lower pole
of uterus .
The fetal heart sounds are best audible on the back( left
scapular region).
AFTER CARE:
