APPLICATION FOR PROBATION

COMES NOW, ACCUSED, ________, through the undersigned counsel,

under the provisions of Presidential Decree No. 968 as amended, most respectfully
state, declare and aver THAT:

1. He is of legal age and detained at Provincial Rehabilitation Center (PRC),

2. On 20 August 2019, he was sentenced to suffer the indeterminate

sentence of Six (6) Months One (1) day to Six (6) years as maximum;

3. The Accused has been detained fro eight (8) months from the time he
was arrested;

4. The crime of which he has been convicted is not an offense against the
security of the state;

5. He has not previously been convicted by final judgment of any offense

punished by imprisonment of more than six (6) months and one (1) day
and/or fine of more than one thousand pesos (P1,000.00);

6. He has not been on probation under the provisions of Presidential Decree

No. 968, as amended.
