He and his brother leaves at Poro, San Fernando, La Union. While their Father, Mother and
Sister leaves at Sto. Domingo Ilocos sur.

Type of Family Structure

Their family type of family structure is Nuclear. It consists of mother who takes care of the
household chores and the father who is the head and breadwinner of the family and together with their
3 siblings that still studying. Their grandmother and grandfather don’t live with them.

Dominant Family Member in terms of Decision-Making in Matters of health care

The father and mother is the one who decides in terms of health status of each member of the
family.
B. SOCIO-ECONOMIC AND CULTURAL FACTORS

His Father’s profession is a doctor same as with his mother. The first and the second child is
studying in their tertiary level taking up Bachelor of Science in Nursing and while the youngest child is
studying at St. Paul college of Ilocos Sur High School Department. They have their own house so they
don’t need to pay house rent. And their family monthly income is approximately P10, 000 per month
just enough to pay for their electricity bill, food and transportation expense.

C.ENVIRONMENTAL FACTORS

The family resides at Poro,San Fernando La Union. Their house is made up of concrete and
metal materials. It has se ven rooms with four comfort rooms. They usually sleep very early at around 11
p.m. and wakes up at 7 a.m. The house is t well ventilated and there is adequate lighting.  Breeding sites
for mosquitoes, flies, cockroaches, and rodents are inevitable due to open garden. Their toilet facility is
located inside the house and a water tank for water supply. The garbage is collected twice a week by a
garbage truck provided by the barangay. They use their two cars for their transportation and they use
cellphones for communication. Their has adequate space for living.

D.HEALTH ASSESSMENT OF EACH FAMILY MEMBER

The father sometimes complains of severe pain at the back of his neck maybe as a sign of
hypertension and over fatigue. The families have a history of hypertension. The family doesn’t use
herbal medicine and goes to the health center when need arises . The family always eats 3x a day and
have a snack in between meals. They always eat vegetables and meat and sometimes fish.

A. Family Structure, Characteristics, and dynamics

The family members are helping each other in the household chores by dividing the work load to
each family member then do it so it will be finished in time. The mother is related to the father by being
her husband then the three kids is their sons and daughter. The father mother and daughter lives at
ilocos sur while the two sons live at san Fernando la union because they are currently studying in a
college. Their family structure is a nuclear type of family because they only live with a father mother and
their children. The father and mother always decides what is proper and in matters of health care they
are both medical in field so there is no need for a problem at health care. Sometimes the family fights in
small things like where’s the charger of the cellphone and the like but they will end their conflicts by
talking with each other in a nice way.

B.Socio-Economic and Culture Characteristics

My father’s job is being a doctor in magsingal district hospital and my mother works at
philhealth. Their income is about P10,000. They give their family’s needs like food, clothes and shelter.
Te mother makes the decision how to save and how to spend the money when they need something.
The father and mother is a college graduate in SLU and the 2 sons is still in college taking up nursing
while the daughter stills studying in highschool. The family is Filipino but the mother is half Chinese and
the children are Chinese in blood.

C. Home and environment

The family has an adequate space for living they have a spacious lot which have 5 rooms to sleep
on. The father mother and the daughter sleeps in 1 room and the 2 sons share the same room. There is
a presence of breeding of insects and rodents but they are taking care of that matter. There is no
presence of hazards. The family has a refrigerator where they can store their foods and their cooking
facilities are complete. Their water source are through deep well and electric water. The toilet facilities
are clean and are flushed type. The garbage are properly disposed and they are being collected twice a
week. The drainage system is all good. The kind of neighborhood they have is congested. Social and
health facilities are seen in their barangay. Their communication is through their cellphones and their
transportations are cars.

D. Values and practices on health promotion/Maintenance and disease prevention

All the family members completed their immunizations. In Ilocos Sur they have their own clinic
so they don’t have any problem regarding health services. They have adequate rest and sleep because
they sleep 6-8 hrs. they always have daily exercise like walking, going to the gym and jogging. Their
relaxation activities are taking a nap and watching T.V. their stress management is by talking to each
other and have a family bonding.
